Screening tools that support whole-person care

Anchored Roots Healthcare uses validated screening tools to better understand symptoms, support care planning, and help patients feel heard.

PHQ-9 Depression Screening

A simple patient questionnaire used to help screen for depression symptoms and track changes over time.

GAD-7 Anxiety Screening

A brief screening tool used to help identify anxiety symptoms and guide the next step in care planning.

Depression Screening & Follow-Up

Screening results are reviewed during the visit and may be used to support counseling, referral, or treatment planning when appropriate.

Medication & Compounding Pharmacy

Compounded GLP-1 medications are fulfilled through Grand Avenue Pharmacy. Medication will be sent to your door. Medication availability, cost, and shipping may vary by dose, prescription, and pharmacy guidance.

When to seek emergency care

Telehealth is a wonderful option for simple, non-emergency needs — but some situations require immediate, in-person care. Please review the guidance below.

If this is a medical emergency, call 911

Call 911 or go to your nearest emergency room immediately. Do not wait for a telehealth visit. For mental health crises, call or text 988 (Suicide & Crisis Lifeline).

Go to the ER or call 911 for:

  • Chest pain, pressure, or signs of a heart attack
  • Difficulty breathing or shortness of breath
  • Signs of a stroke (face drooping, arm weakness, slurred speech)
  • Severe bleeding or a deep or large wound
  • Severe burns, head injury, or major trauma
  • Loss of consciousness, fainting, or seizures
  • Thoughts of harming yourself or others
  • Severe allergic reaction (swelling, trouble breathing)

Not suitable for telehealth:

  • Medical emergencies or life-threatening conditions
  • Severe or rapidly worsening symptoms
  • Conditions requiring a hands-on physical exam, imaging, or in-person testing
  • Broken bones, severe injuries, or wounds needing stitches
  • Controlled substance prescriptions
  • Pediatric care for children under 8 years old
